How to investigate high clamd CPU usage

I have a server with some low traffic sites managed by Virtualmin. The performance is not too well. For quite some time clamd is eating up all CPU.

amavis 28969 106 21.4 301532 217952 ? Rs 15:12 0:04 /usr/sbin/clamd -c /etc/clamd.d/scan.conf --foreground=yes

I am running CentOS 7.

In /var/log/maillog I see quite some messages like:

Sep 11 15:22:13 host2 clamd[29697]: LOCAL: Socket file /var/run/clamd.amavisd/clamd.sock is in use by another process

I am wondering if the high CPU is a result of some misconfiguration. Any advice on how I can investigate or reduce this high load would be nice.
