Steps to reproduce issue
-Go to http://www.virtualmin.com/documentation/tutorial/how-to-create-ftp-account
-Click on 'add new comment' link
-Fill 'subject' and 'comment' fields
-Click on 'Preview' button.
-'Save/Submit' button not showing. Expected result is this button should be display.

I believe the issue there is that the various documentation pages you're looking at there aren't setup to accept comments -- I think only the forums and bug tracker are setup for comments.
So then, you might ask, "why is there an Add Comment button?"
Well, that's a fantastic question, and I'm passing this along to Joe to figure that out :-)
Since I'm not able to add a comment to this doc page myself could you add the following comment to the documentation page?
If you're using Ubuntu Server 8.04 LTS the number of characters for the 'Email Address' field is limited to a total of 16 characters. For example 'username2@domain.com' without the quotes. If you use more than 16 characters the MySQL connection with Drupal might not work.
Same thing could you add the following comment to the documentation page?
At step 2 if you're using Virtualmin version 3.76.gpl GPL or more recent the name has changed to 'Edit Mail and FTP Users'.

There's no hard 16-character limit .. however, as you wrote MySQL only supports up to 16-characters, so any name longer than that will be truncated when creating the DB username. This is usually harmless though.
